What Do I Need To Obtain a Middlesex County Golden Key ID Card?

PRIMARY DOCUMENTS
PROOF OF IDENTITY
ONE OF THE FOLLOWING:

  • Certified Birth certificate from Vital Statistics (If married female, must also present marriage certificate)
  • Valid U.S. Passport
  • Adoption papers
  • Valid U.S. military active duty photo ID
  • Certificate of Naturalization
  • Certificate of Citizenship
  • Permanent resident card (valid)
  • Valid foreign passport with I-551 stamp /valid visa

SECONDARY DOCUMENTS
ONE OF THE FOLLOWING:

  • Social Security Card
  • DD-214 Military Discharge Record

PROOF OF MIDDLESEX COUNTY RESIDENCY
1 OF THE FOLLOWING WITH NAME AND ADDRESS:

  • Business mail issued within the last 90 days
  • Property tax bill, statement or receipt within the last year
  • High school or college report card or transcript from within the past two years
  • Recent tax return form
  • Recent pay stub
  • Valid drivers license

MARRIED FEMALES REQUESTING ID CARDS LISTING A NAME OTHER THAN WHAT APPEARS ON THEIR DOCUMENTATION:

If a married female applicant requests an ID Card containing a name which is different than the name that appears on her documentation (Social Security Card, Voter Registration Card, Driver License, Passport, etc.) she needs to have her name changed on these documents before an ID Card can be issued.

IN ALL CIRCUMSTANCES, APPLICANT MUST PRESENT SATISFACTORY PROOF THAT APPLICANT’S PRESENCE IN THE U.S. IS AUTHORIZED UNDER FEDERAL LAW.

VALIDITY:

  • Cards will be valid for 48 calendar months from date of issue
  • Authorization to be present in the US for a time shorter than the standard periods of such cards, the expiration of ID cards shall be fixed according to the date based on the period in which the person is authorized to be present in the US under federal immigration laws. In such case, it may be renewed if demonstrated that the person’s continued presence in the US is authorized under federal law.

MIDDLESEX COUNTY GOLDEN KEY IDENTIFICATION CARD

If you are age 65 or older, the Middlesex County Clerk’s office issues a Golden Key ID card. Requirements are the same as for a regular Middlesex County ID card.

FEES

Cost of a Golden Key ID card is $10.00

ID applications are accepted from 8:30 AM to 3:00 PM, Monday through Friday. For further information, please call (732) 745-5722.

MIDDLESEX COUNTY VETERAN'S ID CARD

A courtesy identification card is available to all veterans in acknowledgement of service rendered to the United States of America.

Original DD-214 must be recorded with the County Clerk.
Proof of name and address in Middlesex County in the form of a driver's license, business mail, or property tax bill.
Married females requesting ID cards listing a name other than what appears on their DD-214 must also show supporting document showing legal name change.
