How to Use On Line Forms
  • Select the type of grant for which you will apply.
  • Click on the folder that corresponds to your selected category: Mini Grant or Project Grant (which will include Artistic Innovation and Cultural Tourism as options); or General Operating Support (GOS)
  • All the guidelines and forms you need will be contained within the category folder
  • Download the folder and save it to a floppy disk.
  • Work on the grant from your disk. Do not attempt to complete the grant online.

Guidelines are READ ONLY and are found in the file titled GUIDELINES. You cannot alter these pages. You must have Acrobat Reader 3.0 (click here to download Adobe Acrobat Reader) or higher to read the guidelines. However, it is just as easy to refer to the printed version you received in the mail, when completing the application from the computerized, fill-in enabled forms you saved to your disk.

All forms have been designed in Microsoft Word 97. You must have this version or higher in order to maintain the formatting and complete the fill-in enable fields.

When opening a file, if you are prompted to do so - click Enable Macros.

Application narrative and form pages have been designed for best viewing when scaled to 75% or they may be viewed at 100%. Adjust your screen accordingly. No matter which scaling you use, go to the View Menu, click on Page Layout or Page View for ease of completion and to assure formating as designed. It is also suggested that you go to the tools menu, click Options, uncheck all boxes under non-printing characters.

BOXES Boxes can be checked by clicking in the square.
If you make a mistake, a second click will uncheck the box.
BUDGET
INFORMATION
Budgets were created with content boxes equal in width
to the numbers you must insert.
TEXT
INFORMATION
A sufficient number of spaces have been designed into each of the form boxes, including the narrative sections. If you find a name or requested text does not fit within the allotted space, go back and edit the name or text.
MINI
GRANTEES
Large text boxes have been provided for answers to narrative questions. You should have sufficient space to provide DETAILED responses. If you find you do not, attach additional pages that have bee typewritten or word processed.


PRINTING
  1. Complete all questions and forms. If you require additional pages to provide detailed answers, be sure these pages are letter size and have your organizational name in the upper right hand corner. Save your work to your disk.

  2. Insert standard paper into your printer and set on “LETTER” size. Every application page has been designed to print in letter format, that is 8 ˝ X 11.

  3. Print one copy as the ORIGINAL and have the proper authorities sign this ORIGINAL application. Make 10 photocopies of the application – one is for your permanent files.

  4. Submit the ORIGINAL PLUS 9 additional copies to the Commission.

  5. Attach support materials to ALL 10 copies.

  6. Attach the necessary nonprofit documentation to the ORIGINAL only.
NO GRANTS WILL BE ACCEPTED VIA EMAIL.
