Procedure for Requesting Voter Information

Middlesex County has the following process for requesting publicly available voter information:

The ONLY information that may be provided is the following:

	If the voter is currently registered
	Current Address
	Mailing Address (If different)
	Party Affiliation
	Voting History (Which elections they voted in)

All other information is private and may not be released.

Process
The requestor must complete and submit a written request by mail or fax as follows:

Completion of the Form
The form is self explanatory. All information must be completed before we send any information. Then either fax or mail it back to:
